LUCKY DAWG CLASS REGISTRATION:
INTRO TO NOSEWORK LEVEL 1, LEVEL 2 & ADVANCED YOUR INSTRUCTOR FOR THIS CLASS: JAIME FELLOWS, CPDT-KA, CNWI
JAIME FELLOWS, CPDT-KA, CNWI has a broad base of knowledge in dog training, canine
behavior, and animal husbandry. She began training with her own dogs in 1981,
and has been instructing since 1992. Jaime has served as an instructor and
board member for Best Friends Community School for Dogs, and as training director
for National Hearing Dogs of New Mexico. She has worked as a Veterinary Technician
for 8 years. Jaime attends and participates in numerous seminars each year
to increase and refi ne her knowledge of canine behavior, training techniques
and client education. She is active in Schutzhund, a sport which includes
obedience, tracking, and protection work. She holds numerous titles with her
dogs in Schutzhund, as well as AKC events. Jaime serves as an AKC Good Citizen
evaluator. National memberships include Association of Pet Dog Trainers and
United Schutzhund Clubs of America. Jaime is nationally certifi ed by the
independent Certifi cation Council for Professional Dog Trainers. Independently
certifi ed trainers keep up to date on the latest training methods for client
education and research into canine behavior. Lucky Dawg Trainers are experienced
and qualified to not only train dogs, but also to coach and instruct dog
owners.
CLASS AGREEMENT
By completing the form below, I certify that I have read and understand the following:
Lucky Dawg Daycare and Jaime Fellows, CPDT-KA, CNWI will endeavor to create as safe an environment as possible for the training of my dog and will endeavor to offer only sound, safe, and responsible training and training instructions However, I recognize that Lucky Dawg Daycare and Jaime Fellows, CPDT-KA, CNWI is not responsible for any unintentional errors, omissions, or incorrect assertions. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service. Further, I am and will remain responsible for the actions of my dog at all times. I hereby agree to abide by the rules and policies of Lucky Dawg Daycare and Jaime Fellows, CPDT-KA, CNWI training classes as set forth in this agreement and on the preceding pages. I understand that the attendance of dog training classes is not without risk to myself, members of my family, guests who may attend, or to my dog. In consideration of, and as inducement to the acceptance of my application for training membership in this training class, I hereby agree to indemnify and hold harmless Lucky Dawg Daycare and Jaime Fellows, CPDT-KA, CNWI, its officers, directors, instructors, agents, employees, and/or representatives of any and all claims by any member of my family, or accompanying guests of mine of injury, expense, costs, or damages to myself, my dog, or any handler sponsored by me both in class and out of class. In addition, I agree that I will defend and indemnify Lucky Dawg Daycare and Jaime Fellows, CPDT-KA, CNWI for any injury, expense, costs, or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my own actions of the actions of my dog. I have read the above-stated provisions, and agree to accept those responsibilities.
